Discharge Instructions 
 
 
 
 
 

Going Home
Your physician will decide when you are ready to be discharged and will advise nursing and case management. You can anticipate a 1:00 p.m. discharge. When you are prepared to be discharged, please don't forget to pick up any valuables that you may have placed in the hospital safe. If you need to retrieve valuables from the safe, you may do so from 8:00 a.m. to 4:30 p.m., Monday through Friday. Your nurse can assist you in gathering your belongings. Please check to make sure you return home with all items that you had upon arrival to the hospital. Wheelchair transportation is available within the hospital and to your car. A responsible adult needs to be available to provide transportation to your home.

At Home
If arrangements have been made for a visiting nurse or home health professional to visit you in your home, your hospital case manager will let you know.

If home services are not provided as planned, please call (714) 524-4228 and let the case manager know, so that further arrangements can be made.

Should you experience any change in your condition that concerns you or causes alarm, please contact your physician immediately.

Our Emergency Department is open 24 hours a day if for any reason, you cannot reach your physician and you feel you should be seen immediately. Please arrange for a responsible adult to bring you to the Emergency Department or, if necessary, call 911.
