Physician Office Surgery Scheduling Information --
***UPDATES TO “NEW” SCHEDULUING FORM-JULY, 2011 – For offices using the new scheduling form, please note a couple of updates (July, 2011) –
On-Line Surgery Scheduling Form -- Link to New form
1. Patient Information Section -- It is now required to provide a “home phone number” (or some main contact phone # for the patient). The “Home Phone #” field is now in red.
2. Surgery Information Section – The “Special Equipment” title, is now called “Special Equipment/Proctoring/Notes.” Please include proctoring information (if applicable), and any other special notes in this area.
3. At the bottom of the form (after “Scheduling Office #”), a field has been added (that is required to be completed) for you to put your email address. You will now need to provide an email address so we can reply to you with a confirmation and needed information for Placentia-Linda’s Surgery Department. Thank you!

If this is a Worker’s Comp case, we recommend you register the patient using the new form, which has required fields to be submitted with WC cases. Also, if it is required that the physician be proctored, please note that on this form as well (it is already a field on the new form as well).
